What to Bring
To speed up your visit, you can print the forms for your visit from this website and bring them to the office filled out. Please view the drop down menu for this section for a list of the appropriate forms.
For your first visit, you will need to bring your insurance information, if you have insurance.
Details:
In addition to your Drivers license or photo identification, it is very important that you bring evidence of your insurance with you at your first visit. Austin Area Obstetrics, Gynecology & Fertility, P.A. accepts most plans in the Greater Austin Area. Some of these plans include:
- Aetna
- Blue Cross & Blue Shield
- Cigna
- Great West
- Humana
- PHCS
- United Healthcare
- And many more.
If you are unable to bring evidence of your insurance coverage with you for our verification, we will ask you to pay for your first visit and give you a receipt to file for reimbursement with your health plan. We accept cash, pre-printed personal checks, American Express, Visa, MasterCard and Discover. Additionally, we will ask for evidence of your ability to pay that day’s visit at the time you sign in. Once your insurance is verified, we will collect whatever balance is due at the end of your visit. All our patients are required to pay their portion and/or co-pays at the end their visit.
In the event that you are pregnant or are seeking surgery, we will work with you to pre-pay your portion of the obstetrical or surgery fees in advance of either your delivery or surgery date.
